LKMD CUSTOM WEED CUTTING APPLICATION
Custom Cut Procedures:
1. Resident fills out custom cut application below. Information may include name, address, telephone number location and notes.
2. Resident mails check to LKMD PO Box 609 Merton, WI 53056 with application form.
3. LKMD Secretary logs info in Custom Cut 2XXX.xls spreadsheet.
4. LKMD forwards payment to LKMD Treasurer.
5. LKMD Secretary sends e-mail message to Weed Harvesting Contractor notifying them of location of custom cut.
6. Weed Harvesting Contractor executes Custom Cut and sends a return e-mail message to LKMD Secretary when Custom Cut is complete.
7. LKMD Secretary logs completion of Custom Cut.
